Rate of pay: £29,918 per annum full-time
Hours of work: up to 37 hours per week - part-time will be considered on a pro rata basis

SPRING is a partnership of social sector organisations, commissioned by Northamptonshire Health & Care Partnership, to improve the wellbeing of people in Northamptonshire using Social Prescribing.
We take a person-centred approach and a holistic view towards wellbeing. Our goal is to help people to take a leap forward in managing their wellbeing and long-term health conditions.
About the role
Social prescribing empowers people to take control of their health and wellbeing through referral to SPRING Link Workers who take a holistic approach to an individual’s health and well-being. SPRING connects people to diverse community groups and statutory services for practical and emotional support.
As a Link Worker, you will empower people to take control of their health and wellbeing. In collaboration with your clients, you will take a holistic approach that connects people to a range of services for practical and emotional support. You will work collaboratively with the healthcare and VSCE sector to improve local services as well as community and individual resilience and wellbeing.
You will be proactive in encouraging self-referrals within the community, particularly those areas where there is a lower level of healthcare or community service provision. You will be expected to promote positive relationships with our partners and stakeholders: adult social care, hospitals, GPs and other organisations that support older people in the county.
About you
Experience of supporting people with their mental wellbeing, either in a paid, unpaid or informal capacity and f working with the VCSE sector (in a paid or unpaid capacity) is essential. We are looking for someone with experience of data collection and providing monitoring information to assess the impact of services.
You will need to be confident in presenting the programme to community groups and services, so we are looking for someone with excellent verbal communication skills and the ability to liaise with other professionals.
