Dementia Service – Please note that the Dementia Advice Service is for Redbridge residents age 50 plus and their Carers, Family & Friends.

Early Intervention Dementia - The Service provides high quality information, advice, guidance and support to people with dementia and their Carers throughout their journey living with dementia. We focus on early diagnosis and provide support to people in the early stages of dementia and their Carers in line with the National Dementia Strategy objectives. The service provides practical and emotional support on diagnosis, managing the impact of dementia on daily living, increasing choices and improving quality of life. This also involves agreeing the means and level of support appropriate to the needs of the client both before and after diagnosis and working closely with partner organisations. providing information to plan for the future whilst enabling people to live well with dementia.

We also run Living Well with Dementia Groups for people with a recent diagnosis of Early Stage Dementia.

Moderate to Later Stages – The Dementia Service supports people with dementia at various stages and encouraging people concerned about their memory to seek advice and assist them to access appropriate support to maintain their independence, enhance life choices and plan for the future. We also help to develop coping strategies via support planning. This will include signposting and active referrals to relevant services as well as providing information on benefits available and assisting clients to access them where appropriate through various stages of their dementia journey. Providing community based, face to face services for an increased level of support for more complex cases.

The service is provided through office visits, home visits, telephone based support and outreach which includes the Memory Clinic.

Partnership Working

Redbridge Memory Service – We work closely with the Memory Service and provide Advice Surgeries at the Memory Clinic

Redbridge Dementia Partnership – We are members of the Redbridge Dementia Partnership. We work together with other different organisations to improve the lives of people with dementia and their carers living in the local community.

Downshall School Intergenerational Project – With Downshall School, NELFT NHS Foundation Trust, and Lions we are working in partnership to provide an Intergenerational provision that brings together older adults and young children to improve the quality of life and opportunities for both.

Safeguarding
