Bank Take Home and Settle Coordinator
Hours: Bank as and when required covering sickness and annual leave
Salary: £9.95 per hour
Location: East Surrey Hospital or Princess Royal Hospital

We deliver a range of services and activities to meet older people’s needs with our aim being to promote the well-being of all older people, making later life a fulfilling and enjoyable experience and enable people to flourish.
This is an exciting opportunity to be part of a team providing support to vulnerable patients who have been discharged from hospital and provide the support they need to return home safely. Providing practical support in the patients home and referring into other services for ongoing support. No personal care provided.
Nobody wants to stay in hospital for longer than they need to and the current pressure on the NHS to discharge patients as soon as they are medically fit is considerable. Age UK West Sussex, Brighton and Hove can provide this essential support to patients who have been discharged from hospital.
Successful applicants will need a caring, reliable, empathetic and practical approach with good people skills. The role will require a full driving license and use of a car for taking patients home in.
This is a hugely rewarding role for the right person. You will be working as a part of a team that provides invaluable support to vulnerable or older people in the community after being discharged from hospital.
