We are looking for a Bank Crisis Care Coordinator to join our amazing Crisis Team. We work 7 days a week, 8am – 8pm, providing short-term, person centred care to people in later life, who maybe returning home from hospital or for those who find themselves in need. As Care Coordinator, you will work closely with the manager and the other Coordinators of the service, to ensure referrals are processed in a safe and timely way, the rota is covered and carers are well supported.

This is a hugely rewarding role for the right person. You will be working as part of our Crisis team, ensuring safe and efficient delivery of care within a clients home, covering annual leave, training days and sickness.

We are looking to recruit enthusiastic individuals who enjoy working with people to enable them to adopt an independent lifestyle. You will need to be organised with good administration, observation and communication skills.
