About The Role

We are looking for a motivated and flexible individual to join our team as a bank Worker. In this role, you will have the opportunity to work on an as-needed basis, providing support when additional staff is required. This position is ideal for those looking for flexibility in their work schedule while contributing to a dynamic work environment. We are looking for a dedicated and compassionate Bank Take Home and Settle Services Coordinator to join our team and support the Senior TH&S Coordinator in developing and shaping the AUKWSBH Take Home and Settle Services. In this role, you will work closely with the Head of Independent Living to provide assistance to vulnerable individuals, ensuring their safe return home, and facilitating referrals to other support services as needed.

Main Purpose of the Job:

  • Support the Senior TH&S Coordinator in developing and shaping the AUKWSBH Take Home and Settle Services.
  • Collaborate with the Head of Independent Living to provide support for vulnerable individuals, enabling them to return home safely.
  • Refer clients to appropriate ongoing support services.
  • Promote the Take Home and Settle Services within the hospital to meet contracted targets.
  • Receive referrals from hospital staff and assess the appropriateness and safety of taking individuals home.
  • Obtain the necessary discharge approvals from the hospital team.
  • Ensure that individuals are willing to return home and provide any necessary practical support, such as preparing hot drinks and snacks, checking food supplies for the next few days, and confirming their safety and comfort.
  • Position equipment brought from the hospital.
  • Conduct a visual health and safety check to identify immediate risks and minimise them with the client's consent.
  • Refer clients, when appropriate, to the Home from Hospital service for ongoing volunteer support or other relevant organisations.
