Senior Social Prescriber
Hours: 28.5 hours a week (FTC until March 24 with view to becoming permanent)
Salary: £21,182 (£27,500 FTE)

We are the UK’s leading charity for older people and we are passionate about inspiring and enabling people over 50 to Love Later Life. We provide a variety of services and activities within the community, helping older people, their families and their carers by providing opportunities for older people to stay independent, have fun, socialise and build friendships. We also provide services for people over 18, such as our Social Prescribing service.
The Role
We have exciting opportunity available and we are looking to recruit a caring, reliable, empathetic person with a practical approach and excellent communication and people skills to provide support to people referred by their GP practice.
As a Senior Social prescriber, you will have a case load of client’s to support and have line management responsibilities for a team of Social Prescribers. Social Prescribers work in partnership with GP practices supporting clients to navigate their way around Health, Social Care and Voluntary Sector Services to access community based solutions to improve their quality of life.
The role includes providing clients with information to identify solutions to their circumstances and bring about the changes necessary to improve their health and wellbeing.
We are looking to recruit an enthusiastic individual who enjoys working with people to enable them to adopt an independent lifestyle. You will need to be organised with good administration, observation and communication skills.
All roles offer hybrid working, with a mixture of working in GP Surgeries, office and homeworking. This is offered as a fixed term contract until end of March 2024, with a view to becoming permanent, depending on funding being extended.
This is a hugely rewarding role for the right person. You will be working as a part of a team that provides invaluable support to both working age (18+) and older people in the community.
