Community Support Worker (Felinheli Area) 

RESPONSIBLE TO: Community/Home Care Manager
Fixed hours contract – 35 hours per week

GENERAL INFORMATION

Age Cymru Gwynedd a Môn is committed to providing social care and support services of the highest standard and quality to promote the independence, dignity and well-being of individuals to enhance their quality of life enabling them to remain in their own homes.

OUTLINE

To have responsibility in accordance with the practices and procedures of Age Cymru Gwynedd a Môn for the personal and domestic needs of individuals as specified on the service plan. The service aims to ensure the continued independence of the individual in the community by providing appropriate help and support to both individual and his/her informal carers. The initial assessment of the individual’s needs will be undertaken by the Domiciliary Care Manager or Supervisor and a Service Plan outlining the care needs will be drawn up, taking into account the carer’s individual availability relating to hours of work, this will normally be agreed between the Domiciliary Care Manager/Supervisor and the carer. The post is currently Monday – Friday between 9am and 5pm. The post may in the future involve evenings, overnight, weekend and bank holiday working.

MAIN DUTIES AND RESPONSIBILITIES

  1. General domestic services for example cleaning, laundry, making and changing beds, making fires and assist with preparation of meals and any other task which would ordinarily be done in the general upkeep of a home.
  2. Assisting with shopping, collecting pensions, prescriptions and recreation.
  3. General personal care duties which could include assisting individuals in and out of bed, assisting with dressing/undressing, washing, feeding, bathing,
  4. Social duties to include forming a relationship with the individual by listening and talking with the individual and carer and assisting the individual’s contact with family, friends and the community.
  5. To be prompt and on time ensuring reliability and all home visits are for the time allocated. Request the individual’s signature on supplied timesheets at the end of each visit showing the accurate time of arrival and departure.
  6. Report any concerns/changes regarding any individual to the Manager.
  7. Participate and attend staff meeting, supervision, appraisal and training as required by the Manager.
  8. To maintain accurate, concise and up to date records of the individual’s care by recording in the log books provided.
  9. To ensure any documentation such as timesheets and travel expenses are correct and on time.
  10. Confidentiality is to be maintained and respected at all times.
  11. Notify the Manager immediately when unable to carry out services.
  12. The carer may be required from time to time to undertake other duties that commensurate with the post not specifically mentioned in the job description.

The minimum educational /training requirements for the post:

  • QCF Level 2 / relevant experience
  • First Aid
  • Food Hygiene
  • Safeguarding

Specific Training required once in post

  • QCF Level 2 (once probationary period completed)
  • Induction
  • Any mandatory training that is required for the post

3.1 Essential

  • The post holder must hold a full current driving licence with the appropriate business user insurance cover and must have access to a car. Travel expenses will be paid.
  • An acceptable enhanced DBS check.
