A free service supporting older people and adults over 18 who live alone, following a hospital stay.
Support is also available for friends and family of hospital patients.
How we can help
The aim of the service is to prevent people returning to hospital, to boost confidence and physical and mental well-being. We offer up to six weeks free support following discharge from hospital for residents living in the Horsham District..
Following a home visit by our Home from Hospital Co-ordinator we aim to find a volunteer that can provide practical and emotional support. We work with a number of partners who may also be able to support you to rebuild confidence and promote independence.
All our volunteers have a Disclosure and Barring Service certificate and are fully trained. All data we keep is stored according to the latest rules and regulations.
We can assist with these tasks -
Our Home from Hospital staff and volunteers can assist you with tasks such as -
Collecting prescriptions and light shopping
- Collecting prescriptions and light shopping
- Light housework
- Escorted walks
- Engaging in local activities
- Signposting and providing information about other services or equipment you might need
The service is unable to provide -
- Assistance with personal care
- Nursing/Clinical care
- Dispensing medication
- Counselling or advocacy
How much does it cost?
The Home from Hospital service is a free service for up to six weeks and is kindly supported by West Sussex County Council.
How can I make a referral?
You can do this yourself by calling or emailing us (details below) or you can ask a ward nurse, social worker, GP, friend or relative to contact us on your behalf.

Our volunteers provide much needed support, simply chatting and taking an interest in a person’s life.
