Are you looking for a fulfilling way to give back to your community? Join us as a Hospital Discharge Support Volunteer and help individuals transition from hospital to home, ensuring they receive the support they need during this crucial time.
As a Hospital Discharge Support Volunteer, you will be integral in providing reassurance and low-level support to residents of the Isle of Wight who have recently been discharged.
Why we need you?
- Help those who aren't eligible for a social care package. Many patients deemed medically fit for discharge can face extended hospital stays due to concerns about their ability to access support at home, especially if they live alone. While these patients may not require a full social care assessment, they would benefit from additional support to facilitate their return home. Often they remain in hospital due to this - you can help them get home with a safety net in place.
- Reduce the chance of readmission.Your involvement can significantly enhance the recovery and well-being of individuals, ultimately helping to prevent readmission or further crisis.
- Help with the hospital's bed shortage crisis. By providing support and a safety net for these individuals as they transition home, you will directly impact the availability of beds in hospital wards. Facilitating quicker hospital discharges helps free up valuable resources for others in need. With the NHS frequently issuing crisis alerts due to bed shortages on the Island, your contribution becomes even more vital.

What You’ll Need
- Time Commitment: 1-2 hours per week is preferable and based on client needs, however please get in touch to discuss further as there is flexibility in all our volunteering roles. Our volunteers can express preferences regarding tasks, timing, and approaches and we will work with you to make your volunteer experience
- Notice of Changes: You have the right to refuse any request, but once hours are agreed upon, attendance is expected unless exceptional circumstances. Things change - you may decide to increase or reduce your support - all we ask is you give us reasonable notice of this, as clients will rely on scheduled support so we need time to fill this.

How you might be supporting people
- Telephoning or visiting clients at home to ensure they are safe and well.
- Preparing hot drinks and simple snacks, such as sandwiches.
- Checking that utilities are functioning properly.
- Completing small food shops based on client needs.
- Collecting medications for clients.
- Signposting clients to additional services.
- Collecting and/or dropping off necessary mobility aids.
- Escalating any areas of concern to the hospital staff.
Qualities of a Telephone Support Volunteer
- Punctual and committed.
- Positive attitude.
- Ability to remain calm in challenging situations.
- Patience, empathy, and a non-judgmental approach.
- Awareness of when to seek help.
- Commitment to maintaining confidentiality.
- Willingness to complete tasks without direct supervision.
