The home first co-ordinators, run by Age UK Isle of Wight, provide free vital low-level short-term support for patients over 50 - especially those without a support network or elderly couples supporting each other. The team helps people settle safely at home following discharge or provides support in the community to prevent hospital admission.

Support includes collecting prescriptions, shopping, welfare visits and calls, checking heating or home safety and collecting personal belongings if someone is moving into a care setting. The aim is to help individuals remain independent with a safety net in place.

Referrals can be made directly by contacting the team in person, emailing, phone or through the Transfer of Care Hub (TOCH) or HIOWH Community teams.
