Our trained volunteers will visit a person's home following a referral from a health professional.
What does the service offer?
This countywide service is available Monday to Friday, 9am to 3.15pm, excluding bank holidays.
We provide reassurance and low-level support to a person in their own home. An Age UK Northamptonshire volunteer will visit when support is needed.
• Our volunteers will wait with the person when a delay is anticipated for urgent support from local health and social care services
• We can visit to check on someone's welfare
• We support the Intermediate Care Team to assist people who have fallen
• We collect urgent prescriptions and deliver to the patient
• We collect and deliver low-level mobility equipment
This is not an emergency service.
Who is it for?
We accept referrals from the Intermediate Care Team or Remote Monitoring Hub for people living in Northamptonshire.
Follow up and wraparound support
Our Coordinators will contact the person 24 to 48 hours after the initial visit has been completed to explore any longer-term support needs. They can link people with longer-term support services such as Collaborative Care Team, Ageing Well and other partner organisations.
