Social Prescribing Coordinator, 35 hours

Linking clients to services in their community

Monday to Friday Salary £19,692 actual (20,817 FTE)

Based in East Grinstead

Age UK West Sussex, Brighton and Hove deliver a range of services and activities to meet vulnerable people’s needs. Our aim is to promote the well-being of all older people and make later life a fulfilling and enjoyable experience and enable people to flourish.

We are delighted to be working in partnership with the Primary Care Networks communities (PCN) throughout Horsham & Mid Sussex (formally Communities of Practice) and we are looking to recruit a Social Prescriber to cover the East Grinstead area. We are looking for and enthusiastic individual, who enjoys working with vulnerable people to enable them to adopt an independent lifestyle. You will need to be an organised person with good administration, observation and communication skills.   

The Social prescriber will work within a multidisciplinary team supporting clients to navigate their way around Health, Social Care and Voluntary Sector Services to access community based solutions to improve their quality of life.

The role may include working with volunteers to provide clients with the information to identify solutions to their circumstances and bring about the changes necessary to improve their health, wellbeing.  They will support the Senior Social Prescriber to run peer support groups.

This post requires you to have a full driving licence and use of a car.

Crisis Carers – Brighton and Hove

Are you able to help you local community by becoming a key worker? Do you have a passion to care for older people? Are you looking for a flexible carer, supporting older people when they most need it? Is you current role at risk due to COVID 19, being furloughed or made redundant? Or are you an ex carer looking for further rewarding challenges?

Role Responsibility

We are looking for new bank colleagues to join our AMAZING Crisis Care team. We support older people when they need it most. We work 7 days per week and our service operates 8am – 8pm daily. Hourly rates start from £9.94 per hour (week day) and increase to £11.95 for Sunday/Bank holidays. We also offer paid travel, admin and training time as well as travel expenses for our Carers. The service provides short term care (up to 14 days) for our clients following hospital discharge, onset of a sudden illness or when informal care breaks down.

Additional information

  • We offer a variety of shifts 7 days a week
  • No experience needed as full training provided

Home Helpers Required


We are looking for SELF EMPLOYED HOME HELPS in the BRIGHTON & HOVE and WEST SUSSEX areas to help with household tasks, cleaning, shopping and provide social support to assist older people to remain independent in their own homes. Working between Monday and Friday, the hours are flexible and according to your availability.  Rates of pay are between £11 and £15 per hour depending on the area.

Car drivers preferred but not essential.
