A multi-agency, multidisciplinary teams delivering a preventive service across West Sussex. The team includes health advisors (qualified health professionals); social care workers and support workers from Age UK West Sussex based in the North and West of the County and Guildcare in the South of the County, who can advise over the telephone or come and see you in your own home. Their objective is to improve quality of life and the promotion of health and wellbeing.

Our service is for adults and older people who might not be eligible for other statutory services and who have unmet physical, psychological, social, functional and/or environmental needs.

We provide information provision, education, advice and support, to enable independence and self-care, and undertake proactive interventions and problem solving. This reduces the risk of the need for reactive, more intrusive and costly interventions at a later date.
