Community Responders Coordinator
Location: Based at Lotus House, Victoria Street, Northampton NN1 3NR (Countywide Service)
Rate of pay: £18,295.68 per annum (FTE £28,205.84)
Hours of work: 24 hours per week - days and times to be confirmed
About the role
Our Community Responder Volunteers Service, in partnership with Northamptonshire Healthcare Foundation Trust and the Integrated Care Board, supports vulnerable people throughout Northamptonshire. Working with the Intermediate Care Team (ICT) and Remote Monitoring Hubs to provide low level support to people in their own homes to prevent unplanned hospital admissions.
As Community Responders Coordinator, you will work in a task centred way primarily with Community Health professionals (virtual care team & community nursing teams) who identify individuals suitable for the service. You will be responsible for:
- Coordinating referrals into the service
- Identifying and deploying a suitable volunteer into the community to carry out the assigned task
- If an appropriate volunteer is unavailable or additional support is needed you will be required to attend community support visits
- Triage and assess incoming referrals directing them to the most appropriate team if volunteer support is not appropriate
- Work closely with our community teams and our partners within health, social care and other VCSE organisations
- Support the Senior Coordinator with ongoing recruitment and management of a team of volunteers across the county.
You will promote the work of Community Responders and Age UK Northamptonshire throughout Northamptonshire and will be responsible for developing and maintaining good working relationships with health professionals and other service providers. You will be responsible for ensuring all patient identifiable information is stored and disposed of appropriately, and that Information Governance and Confidentiality procedures are adhered to.
About you
You will need to be able to absorb information and plan ahead, working in a systematic and organised way. You will need to be able to think quickly on your feet and be able to use your experience an initiate to decide a best course of action in challenging times. You will be motivated by challenge and enjoy seeing problems through to their solution, knowing that your timely actions have improved life for someone else.
Good knowledge of community support services is desirable, alongside experience of care work or a professional qualification in Health or Social Care. If you thrive on being flexible and meeting new people, we would love to hear from you.
The role may require you to complete community visits therefore you must be a car driver with access to your own vehicle.
