Connect North

Many things affect our health and wellbeing. Social factors such as work, money, housing problems, the challenges of managing long-term conditions or feeling lonely or isolated are just as important to our health and wellbeing as our physical needs.

Connect North: Your Pathway to Wellbeing can help to connect you to the right help and support to address these social needs.  This is sometimes called “social prescribing” and our aim is to improve your wellbeing.

Connect North is a holistic Social Prescribing service for the Northern Trust area which aims to support adults aged 18 years and above and living in or registered with a GP in the NHSCT area who are experiencing social, emotional or practical issues which are affecting their health & wellbeing and require assistance from a link worker to determine appropriate supportive services.

Connect North is delivered in partnership with the Northern Health & Social Care Trust, Age NI, Building Communities Resource Centre and Mid and East Antrim Agewell Partnership.
