Connecting Carmarthenshire Wins Prestigious Social Care Accolade

We are proud to announce that Connecting Carmarthenshire has been awarded the Social Care Accolade for Working in Partnership at yesterday’s prestigious ceremony.
This recognition is a testament to the shared commitment and dedication of everyone involved in delivering the Connecting Carmarthenshire service. The Working in Partnership category received the highest number of nominations, making this achievement especially significant.
The judging panel praised Connecting Carmarthenshire for its diverse and impactful range of services, delivered through strong and effective collaboration. They made special mention of the honest and transparent approach taken in addressing challenges and overcoming barriers to partnership working—highlighting the positive outcomes this has generated for citizens across the county.
A certificate will be issued to each Connecting Carmarthenshire service partner, as well as to every Micro-Commissioned project, in recognition of their valued contribution to this success.

Connecting Carmarthenshire is a collaborative, community-focused initiative designed to improve wellbeing, reduce loneliness and isolation, and connect people with local services, support, and each other. The project brings together a broad range of partners from the third sector, local authority, health services, and community groups to offer tailored, person-centred support to individuals in Carmarthenshire.
Through a network of Micro-Commissioned projects and service partners, Connecting Carmarthenshire empowers citizens to access help in a way that suits them—whether that’s through befriending services, wellbeing support, practical assistance, or social activities. As a key partner in this collaborative project, Age Cymru Dyfed provides tailored information, advice, and guidance to help individuals access local services and community support that meet their unique needs.
