Dementia Wellbeing Connectors attend Ceredigion Womens Institute wellbeing event

Last week our Dementia Wellbeing Connector staff Lucy and Emma attended Ceredigion Women's Institute Health and Wellbeing event with a stand, presenting on the Dementia Wellbeing Connector project and taking Q&As.
Enhancing collaboration and care across communities
As part of All-Wales Dementia Care Pathway of Standards, Age Cymru Dyfed delivers ‘Standard 12’ where newly diagnosed people living with dementia get a named point of contact (connector) in the Health Board footprint, from diagnosis through to end of life or admission into care. Dementia Wellbeing Connectors are responsible for coproduction of a wellbeing plan and ongoing support to individuals living in Ceredigion, Carmarthenshire and Pembrokeshire.

Others attending - Frailty Team, dietician from Hywel Dda Health Board NERS and Nursing Faculty staff from Aberystwyth Uni, plus Age-Friendly Ceredigion from Ceredigion County Council.

How we make a difference
- 96% of people living with dementia (PLWD) report they feel empowered to speak for themselves and contribute to decisions affecting their life
- 94% of PLWD feel in control of and /or supported with their wellbeing plan
- 100% of PLWD felt listened to and treated with empathy, dignity, and respect, and see their quality of life as optimal given circumstances and health and feel adequately supported when accessing and navigating the health and social care system.
