Introducing the Regional Dementia Connectors - Enhancing Collaboration and Care Across Communities
As part of All-Wales Dementia Care Pathway of Standards, Age Cymru Dyfed delivers ‘Standard 12’ where newly diagnosed people living with dementia get a named point of contact (connector) in the Health Board footprint, from diagnosis through to end of life or admission into care.
Dementia Wellbeing Connectors are responsible for coproduction of a wellbeing plan and ongoing support to individuals living in Ceredigion, Carmarthenshire and Pembrokeshire.

How we make a difference
- In the last quarter, 96% of people living with dementia (PLWD) reported they felt empowered to speak for themselves and contribute to decisions affecting their life
- 94% of PLWD felt in control of and /or supported with their wellbeing plan
- 100% of PLWD felt listened to and treated with empathy, dignity, and respect, and saw their quality of life as optimal given circumstances and health and feel adequately supported when accessing and navigating the health and social care system.
