About us
Age UK Isle of Wight is an independent charity that offers support and services to older people and families suffering with a Dementia, encountering memory loss or waiting for a diagnosis. Our services empower individuals, supporting their health and wellbeing through the provision of information, signposting & advice, and through income maximisation.
About The Role
Our OPMH (Older Persons Mental Health) navigators provide a single named OPMH Navigator for the South, North East or West & Central localities. To support people living with Dementia or suspected Dementia, their carers, Social Care Colleagues, and GPs to be able to support timely access to information services and support, with continuity of care and the building of a sustainable professional relationship.
Establish effective liaison pathways between primary care (based in the three localities), secondary care and third sector providers.
About you!
If you have the following attributes then we would like to hear from you:
- A background in health, social care and/or voluntary sector and experience with working with older people with Dementia, health and welfare concerns
- Confident communication skills and excellent interpersonal skills
- Concise and accurate written skills and proficient IT skills
- Ability to manage complex workloads and work within a multi-disciplined team
- Self-motivation and a positive approach to work
- Valid Drivers License and vehicle
