Hospital Discharge Research Report

Age UK Kensington and Chelsea is proud to publish The Realities of Hospital Discharge Report – Conversations with people living with dementia and their carers on their recent experiences of the hospital discharge process in Kensington and Chelsea.

This report draws from over 25 interviews over a six month period and is based on and amplifies the recent first-hand experiences as told by the participants themselves, as well as what the first few months post-discharge was like for them.

Alongside stories of kindness and good practice, the report reveals a systemic lack of a joined-up approach, inconsistencies across the health and social care sector and often a lack of a person-centred approach.

There is an audio report embedded in the report so you can hear the actual voices of the older people and their carers in their own words.
