Dementia Wellbeing Connector Carmarthenshire

  • Salary: DP3(1) - £25,298
  • Hours: 35 hours per week
  • Place of Work: Hybrid working model from home and Llanelli office. Travel
    throughout Carmarthenshire to client homes.
  • Fixed term contract to 31/5/2027 with possible extension, subject to further
    funding.

We are looking to recruit a self-motivated, enthusiastic, hard-working team member to join our exciting new service that will support people living with Dementia (PLWD) and their carers. The service aims to provide a full wrap-around support that will assist the PLWD in navigating their individual journey and increase their overall wellbeing as much as possible.

Volunteer Coordinator Dyfed

  • Salary: DP2 (1) – DP2(5) £24,060 - £25,250 
  • FTE (£14,436 - £15,150 actual)
  • Hours: 21 hours per week
  • Place of Work: You will be working from one of the Age Cymru Dyfed Offices, with the need to travel to different sites on occasion.

We are looking to recruit a Volunteer Coordinator to support the delivery of a comprehensive volunteering programme on behalf of Age Cymru Dyfed, to include the promotion of a diverse range of volunteering opportunities and the recruitment, induction, development and support of volunteers throughout the organisation.

Housing Support Officer Pembrokeshire

  • Salary:Band DP2 £24,060 (£14,436 actual)
  • Hours: 21hrs per week
  • Place of Work: The post is based in our Haverfordwest office

We are seeking a self-motivated, enthusiastic, and hard-working Housing Support Officer to join our dedicated team. As a key member of our organisation, you will provide targeted support to older people across the county, focusing primarily on housing-related needs. Your work will involve a combination of home visits, community outreach, and liaising with external agencies e.g. Department of Work and Pensions, to ensure individuals receive appropriate and timely support. You will also be responsible for providing accurate and relevant information to help clients understand and access the services and benefits available to them ensuring older
individuals receive the support they need to live safely and independently.

Housing Support Officer Ceredigion

  • Salary: Band DP2 £24,060
  • Hours: 35hrs per week full time, and job share will be considered
  • Place of Work: working from the Aberystwyth Office, as well as being home based, and travel throughout Ceredigion is essential
