Dementia Wellbeing Connector Pembrokeshire

  • Salary: DP3(1) - £25,298
  • Hours: 21-35 hours per week
  • Place of Work: Hybrid working model from home or the Haverfordwest office. Travel throughout the county to client homes.
  • Currently until 31 March 2026 potential for extension if funding allows

We are looking to recruit a self-motivated, enthusiastic, hard-working team member to join our exciting new service that supports people living with Dementia (PLWD) and their carers. The service aims to provide full wrap-around support that will assist the PLWD in navigating their individual journey and increase their overall wellbeing as much as possible.
