Do you want to work for a caring and forward-thinking charity?

If your answer is yes then we have the perfect role for you!

At Age UK Kensington and Chelsea we are passionate about our people, we care about the service we provide and empower our staff to deliver great outcomes for those who come into contact with our services.

About the role

Our My Care My Way service is a great example of this, bringing together health and voluntary sector expertise to deliver real change to how care is delivered in West London. We have collaborated with local NHS partners to offer a unique integrated care service that was at the forefront of Social Prescribing development in London.

MCMW aims to provide information, navigation and support to patients where it is most needed. Our Health and Social Prescribing Co-ordinators are aligned to GP surgeries in our local community and work alongside specialist nurses to provide additional care to older patients.

Each HSPC will care plan for a caseload of patients, contacting individuals at a frequency based upon need and building a relationship over time. With a focus on the wider determinants of health - such as level of physical exercise, social connection and one’s environment - our HSPCs link patients into relevant services and activities that will improve their lives in the longer term.

We have a fantastic staff benefits package and we provide annual training and development opportunities.

About us
