If you would like a printed application form or would like your application and documents checked, Age UK Dudley will assist in helping you fill the forms out and send the completed documents back to Centro - WMPTA, where it will be processed. There is a £2.00 charge to cover the cost of administration.
New passes will be sent directly to people`s homes within four weeks of applying.
What you will need when applying for a older peoples bus pass
- One recent passport photo
- Proof of age (for e.g. retirement pension book, driving licence, passport or birth certificate, if the name on your birth certificate is different please also provide proof that your name as changed, such as a marriage certificate)
- Proof that you live in the West Midlands (for e.g. retirement pension book, driving licence, council or housing association rent book, council tax bill or gas/electricity bill from the last 12 months)
