Pathways 4 Life is a joint partnership scheme between Age UK Walsall and Accord Housing Group providing a wide range of advice and support for people with a diagnosis of dementia, anyone who may have a general query about dementia, friends, families and carers of people with dementia.

The service is free to access and is funded by Walsall Council and Walsall Clinical Commissioning Group.

The scheme comprises:

4 x Dementia Support Workers - working within the Manor Hospital to support patients with dementia both on the wards and within A&E.

6 x Personal Assistants in Dementia - supporting people post diagnosis with a range information, advice and signposting.

1 x Dementia Support Worker, Hard to Reach Groups - supporting people with dementia, or may be worried about memory loss, from a variety of harder-to-reach communities

1 x Dementia Cafe Coordinator - delivering 7 cafes per month in a variety of locations across the borough, plus one cafe at the Manor Hospital every week.

2 x Dementia Support Workers - Care Homes and End of Life (in partnership with St Giles Hospice)
